The CHOOSE 180 Aftercare program ensures that young people remain supported, connected, and empowered long after they complete one of our programs or transition back into their communities.

Through holistic case management, we meet young people where they are, providing guidance, resource navigation, and a safety net to help each participant to build stability. We host empowerment-focused workshops and create space for young people to reflect, grow, and build real life skills that help them take ownership of their story, disrupt cycles of harm and step into a future rooted in healing and hope.

For those who do, our team is ready.

Many CHOOSE 180 participants continue to navigate housing, employment, education, and mental health challenges even after their court cases are dismissed. Our Aftercare team walks alongside these young people, offering consistent, holistic support and connection to resources as they build toward stability and healing.

Eligibility: Aftercare is offered to any young person participating in a CHOOSE 180 program.
