Aftercare

The CHOOSE 180 aftercare team builds relationships with program participants and provides personalized, comprehensive support for up to a year after coming to our programs. Support could mean mentorship, access to resources, referrals to specialized organizations, and more. The type of support is different for each young person and solely based on their needs and goals.

Not every program participant needs ongoing support. For those who do, our team is ready.

Aftercare services can include:

  • Counseling and expressive arts therapy services

  • Food and rent assistance

  • Referrals for other resources such as substance abuse, tutoring, etc.

  • Assistance with job readiness 

  • Personalized support and goal setting based on needs and interests

Eligibility: Aftercare is offered to any young person participating in a CHOOSE 180 program.
